This part is a 2 1/4" Wide cap  1"slip x 1"slip x 1"slip Notched Spa Small Diverter Complete Assembly Gray Valve. 

HTPX279HLSThis Valve can be found on a few brands but is a very rarely used valve... You may have a Waterfall Control valve that looks like this on the top but only has (2) 1" Slip Fittings look in the Waterfall section for that valve.. Compare the measurements to be sure this is correct.

HydroAir "Hydroflow" 3-Way Valve
1" sizes topside mounted easy to turn, allows for zoning to banks of jets and permits variable flow to different areas. Hole size for 1" valve is 2-1/16"..
